Patient IO

Patient IO was a care coordination platform that helped health systems transition to value-based care by better connecting providers with patients between visits. The platform enabled providers to engage patients and their caregivers outside the hospital setting through remote monitoring and communication tools. Patient IO was acquired by athenahealth in August 2016 to bolster its care coordination capabilities.
