HealtheMed is a Minnesota public-benefit corporation providing Clinic@Home, a technology-enabled care model for people receiving Medicaid waiver services. It combines daily engagement, remote vital-sign monitoring, telehealth, medication support, and a dedicated care team to help clients with complex or chronic conditions remain stable and independent at home. The service runs on an in-home display, tablet, or mobile app and can connect Bluetooth health devices. HealtheMed coordinates with case managers, providers, and existing waiver services, seeking to identify health changes early and reduce avoidable emergency-room visits and hospitalizations.
